What is CDPAP?
Consumer Directed Personal Assistance Program (CDPAP) allows patients to take full control over their home health care by hiring a caregiver they choose and have Medicaid pay them for it. The caregiver can be anyone a patient chooses – even a child, relative, neighbor or friend. CDPAP is a personalized alternative to traditional home care, so you can enjoy the comfort and the lifestyle you’re accustomed to, under the care of someone you know and trust and who is offering the service you need at the hours you decide. Enjoy the peace-of-mind of being cared for by someone you have chosen, who you know and trust to ensure your health.
What We Do?
We walk you through the entire process and take care of all of the paperwork for you. Our caring and knowledgeable intake specialists work with you to apply for the CDPAP program on your current Medicaid managed long term care (MLTC) insurance plan. And if you have straight Medicaid or are on a regular managed care plan, we will walk you through the process of choosing the best MLTC for you. And if you’re not already on Medicaid, our team will help you determine if you are eligible for Medicaid and, if you are, will assist you with the Medicaid application process.
Who is CDPAP For?
If you have physical disabilities and/or a chronic illness leaving you in need of in-home care, our program allows you to choose who cares for you and have Medicaid pay them for it. That way, they can devote themselves to caring for you without having to worry about supporting their own family.
Benefits
Choosing Your Own Caregiver: Under New York Law, you have the right to choose the caregivers that you allow into your home. This gives you the freedom to select the person entrusted with your health from your own social circle.  You can choose as many caregivers as you feel necessary for your health.

Customized schedule: Personalization is the key to CDPAP, allowing you to create a schedule which suits your unique habits and responsibilities without upsetting your caregiver’s schedule.

Payment that Suits Both Parties: You may want to select a family member or friend as your caregiver, but without CDPAP, that person may not be able to afford missing time from work. By signing up for FreedomCare’s CDPAP program, you can hire the person you choose and Medicaid will pay the person you choose rather than a random home aide from an agency. This way, the caregiver you choose will be free to devote their full attention to your care.

Complete Control Over the Terms: Should you decide, after beginning care, that you want to switch from one caregiver to another, you are free to fire one caregiver and hire another one. The choice of who cares for you and what hours they work always remains in your hands.

Formal Compensation for Caregivers: Our program offers fair payment so that your caregiver can earn more than the minimum wage during their care for you. FreedomCare also offers patients the option of giving their caregiver a bonus at the end of the year if they are satisfied with their service.

How to Qualify
Medicaid: To take part in this program as the one receiving care, you must be eligible for New York State Medicaid.

Doctor’s Order: Managed Long Term Care (MLTC) insurance companies sometimes require a form filled out by your doctor stating that you need home-care and that your spouse is unable to care for you. FreedomCare will assist you with this entire process.

You or a Designated Representative Direct Your Care: The patient has the right to choose his or her own caregiver if they are capable of directing their own care. If a patient is unable to direct their own care, his or her Medicaid designated representative will decide who to choose as a caregiver under FreedomCare’s CDPAP program.

Assessment by Your Local Social Services: FreedomCare will work with you on your application for CDPAP. Once we get the ball rolling, your MLTC will send a plan assessment nurse to evaluate your health care needs and decide how many hours of care per week you are entitled to.

Once Care Gets Started
Work Schedule Management: We will manage the hours your caregiver works for you and assist him or her with schedule and details you have established with your caregiver regarding time-off and leave.

All the Support You Need: We are available anytime to discuss any question you have about the CDPAP program, your caregiver, or anything else. If your caregiver needs assistance using our timesheet app for his or her smartphone or with submitting paper timesheets, we are here to help.

Payroll: We will process the weekly payroll to ensure your caregiver is well compensated for his or her time and effort.
